What is tesamorelin?

Tesamorelin is a synthetic analog of growth hormone–releasing hormone (GHRH). It is a 44–amino-acid peptide modified with a trans-3-hexenoic acid group that makes it markedly more stable than naturally occurring GHRH, slowing breakdown by the enzyme dipeptidyl peptidase-IV (DPP-IV). It first reached the market in November 2010 under the brand name Egrifta® (later Egrifta SV®), developed by Theratechnologies and approved by the FDA to reduce excess visceral abdominal fat in people with HIV-associated lipodystrophy.

That approval matters for one reason in particular: tesamorelin is the only GHRH analog with full FDA approval. In a research market full of compounds that have never been near a regulatory file, that gives it an unusually deep, well-documented clinical record to study.

How tesamorelin works

Tesamorelin binds GHRH receptors in the anterior pituitary, prompting the gland to release the body's own growth hormone (GH). Crucially, it does not supply exogenous GH the way injected synthetic HGH does. Instead it amplifies the body's natural, pulsatile GH rhythm, which in turn raises insulin-like growth factor 1 (IGF-1) downstream. Researchers studying the GH axis are interested in tesamorelin precisely because it preserves that pulsatility rather than flattening it.

What the research has investigated

The pivotal clinical program used CT-measured visceral adipose tissue (VAT) as its primary endpoint — a far harder, more specific measure than BMI or waist circumference. A landmark 2010 New England Journal of Medicine trial reported that tesamorelin reduced visceral adipose tissue by roughly 15–18% over 26 weeks versus placebo in subjects with abdominal fat accumulation, alongside changes in IGF-1 and lipid markers.
